- Willis Claim Form 2010
- Please find attached a copy of the new Willis Claim form which needs to be used going forth. Maximum payout now €4,500 and excess raised to €100. Membership number will be needed so nothing we can do for a player who has not paid membership. It is the responsibility of the Team Manager/Selector to make sure that the referee has details of the injured player after a match. The Player’s Welfare Officer must also be notified with relevant details of an injury as soon as possible (no more than 2/3 days after injury)
